Well-Being Check (WHO-5)

Five quick statements about the past two weeks. Good for a gentle check-in even when nothing feels "wrong."

Please say, for each statement, which is closest to how you have been feeling over the last 2 weeks.

  1. I have felt cheerful and in good spirits
  2. I have felt calm and relaxed
  3. I have felt active and vigorous
  4. I woke up feeling fresh and rested
  5. My daily life has been filled with things that interest me

About this tool

The WHO-5 Well-Being Index is a short questionnaire from the World Health Organization. Instead of asking about problems, it asks about positive states: cheerfulness, calm, energy, rest and interest in daily life.

Each statement is scored from 0 to 5. The raw total (0 to 25) is multiplied by 4 to give a percentage score from 0 to 100. A score below 50 suggests low well-being and is commonly used as a prompt to look further. A score of 28 or below is often associated with depression.

What to keep in mind

  • It is a screening tool, not a diagnosis. Only a qualified professional can diagnose a condition.
  • It reflects how you have felt recently, not who you are, and results can change.
  • It cannot replace an evaluation that considers your history, health and circumstances.
